Health Requirements

Below are health requirements that need to be submitted to Student Health Services if you plan on attending University of Bridgeport.

If you do not provide Student Health Services with the required form completed, you will not be able to register for classes or reside in on-campus housing.

Please use the appropriate form for the program you are entering.

Health Form A - Mandatory for all Undergraduates, Health Sciences Students, and International Students

Health Form B - School of Professional Studies, Domestic Graduate Students, Not including Health Sciences

School of Nursing and PA Institute: Health Screening Form - For Nursing and Physician Assistant students

School of Nursing and PA Institute: Immunization History Form - For Nursing and Physician Assistant students

TB Risk Assessment Form - In lieu of the PPD or IGRA requirement, this form can be filled out and submitted.

Please submit your required forms, including any additional vaccination records as soon as possible via in person or by uploading to the Submit Health Forms tab at the top of the page (unless otherwise instructed). Please include your Student ID number on ALL documentation, and ensure ALL documents are legible.

Student Athletes

Student Athlete Pre-Participation Physical Exam Form (PDF) – For students in any program that are joining a NCAA Intercollegiate Athletic Team. This form is required prior to participation in any varsity sport. This does not include Intramural, Recreation or Club sports.

Students will be responsible for obtaining these vaccines from an outside provider if necessary. Due to the high cost of these vaccines, please discuss these requirements with your primary care provider or your local health department.

Hepatitis B

A law enacted by the State of Connecticut mandates that all college students must be informed of Hepatitis B infection and its devastating consequences. All Physician Assistant students are required to be vaccinated for Hepatitis B or provide serologic evidence of immunity. College students are at an increased risk of developing a Hepatitis B infection. All students are strongly encouraged to be vaccinated for Hepatitis B, which is administered as a series of three (3) injections over a six-month period. Refer to the Hepatitis B Fact Sheet (PDF).
