An accident insurance fee is charged to all full-time students and may not be waived.  To waive the mandatory insurance domestic students must submit a waiver form by March 1 2007 for those starting in Spring 2007.  For more helpful student insurance information and links to to the UB Bursar's insurance page.

Immunization and PPD Requirements
Connecticut law requires that all college students born on or after January 1, 1957, to submit documentation of two separate measles vaccinations and one rubella vaccination. The first measles and rubella vaccine must have been given on or after student's first birthday and after January 1, 1969. The second measles vaccine must have been given after January 1, 1980. Health Services has the measles and rubella vaccines and will be happy to administer them to students. Connecticut law mandates that students who reside on-campus must provide documentation of the meningitis vaccine. If a student is vaccinated at Student Health Services, the cost for meningitis vaccine  must be paid by cash or check at the time of the vaccination.

A PPD (test for Tuberculosis) is required of all new students within 6 months of registration to the University.  If the student has a history of a positive PPD documentation of a negative chest X- ray and a letter from their physician regarding whether prophylactic therapy was initiated or refused is necessary.

A new law enacted by the State of Connecticut mandates that all college students must be informed of Hepatitis B infection and its devastating consequences. College students are at an increased risk of developing a Hepatitis B infection. All students are strongly encouraged to be vaccinated for Hepatitis B, which is administered in a series of three injections over a six-month period. Refer to the Hepatitis B fact sheet.

Health Examination Form

A Health Examination Form is required for all students with the exception of Part Time Graduates and IDEAL students.  This form should be filled out by the student's primary physician and submitted to Student Health Services prior to registration and/or admission to the residence halls.  Immunization and PPD requirements are also listed on this form.

Students joining an Athletic team must also submit a Sports Physical Form completed by their physician.
