INSTRUCTIONS FOR COMPLETING THE ONLINE APPLICATION(ACUPUNCTURE)

Before filling out the Online Application for the Acupuncture Institute, please carefully read the instructions that follow.

• Please complete the online application no later than 30 days prior to the beginning of the semester you which to be admitted.
• Be sure to fill out all parts of the Online Application. Optional information is labeled as such.
• When you have completed each page of the application, click "Submit" or "Next" and you will be taken to the following page.

In addition to the application all applicants must submit the following:

• $50.00 non-refundable Application Fee is required.(Check or money order made payable to the University of Bridgeport.)
• Official college transcripts from all colleges or universities* attended, indicating degree(s) earned and date(s). Undergraduate transcripts must show clear evidence of the capacity for independent study and the ability to pursue graduate work.
• Official course descriptions for all science prerequisites taken. Official catalogs and/or syllabi may be requested.
• Two (2) letters of recommendation to be sent directly to the Director of Admissions, Acupuncture Institute by individuals who have known you for at least one year. These should include at least one licensed health care professional or one faculty member. Letters from close friends or relatives are not appropriate.
• Complete a Personal Statement addressing the following questions (space provided in the Online Application):
  o Why do you want to be in the allied health field?
  o Why have you chosen Oriental Medicine?
  o How have you prepared for this rigorous academic effort?

* All transcripts from international institutions must be evaluated by an approved credential evaluation service.

Important Note for International Students:

International Students should only fill out the Application for International Students. There is no need to fill out any other application.
