Student Accessibility Services (SAS) engages in an interactive process with each student and reviews requests for accommodations on an individualized, case-by-case basis. Depending on the nature and functional limitations of a student’s documented disability, he/she may be eligible for reasonable and appropriate accommodations. If you have a documented disability and are seeking accommodations please follow the steps below:
STEP 1: Please complete the information on this form and send to Student Accessibility Services (SAS).
STEP 2: Please familiarize yourself with the Guide for Students with Disabilities.
STEP 3: Please have the treatment provider(s) send all the testing records and treatment summary, including specific diagnoses and how specifically these diagnoses affect your ability to function academically or otherwise. Specific academic accommodations must be recommended by the provider based on this information. This information cannot be more than 3 years old.
STEP 4: Once the form/records are submitted to Student Accessibility Services (SAS), an appointment must be made to review the documentation submitted.
STEP 5: An accommodations letter will be provided to professors on or prior to the first day of class.
STEP 6: Each professor must sign the letter and you must return it to Student Accessibility Services (SAS).
